Our office is located behind the Holiday Inn Hotel off of 10th Ave S. From 10th Ave S turn South onto 4th Street S. Head straight for 1.5 blocks. As you start to reach the top of the hill take the SECOND left turn into the parking lot. There is a sign at the turn into the parking lot that says Prospect Heights Medical Center.
